Asuncion "Susan" Prats Obituary

Asuncion "Susan" Prats, 82, of Woonsocket, passed away peacefully at home on June 20th, 2025 with her two loving daughters by her side. Born June 11th, 1943 in New York City, she was the daughter of the late Jose Prats and the late Alicia (Sanchez) De Moya.

Susan worked as a doctor's assistant for a medical center in Brooklyn, NY. She was a devoted volunteer at Community Care Alliance in Woonsocket. Susan formed deep and lasting relationships with the clients she served. She was passionate about her work, known for her caring and giving nature, and beloved by all whose lives she touched.

She especially loved taking road trips and spending time at the lake with her entire family. Those moments brought her the most joy. The Autumn Fest was an activity she looked forward to every year to have a Gyro with her daughter Jeanne. Susan found happiness in life's simple pleasures: enjoying breakfast outings, indulging in seafood, and treating herself to pedicures. She also had a love for plants and they could be found all over her apartment.

She is predeceased by her daughter, Iliana Ojeda-Rivera, she is survived by her son, Francisco Ojeda Jr. and his wife Adele of Hubbardston, MA; her daughters, Jeanne and Susan Ojeda, both of Warwick; fourteen grandchildren; twenty three great grandchildren; and two great great grandsons, last but not least her "baby" and best friend, Lola, her dog. She was the sister of the late Joseph Prats and the step daughter of the late Raymond De Moya.

Tragically, on the same day Susan passed, her 33 year old granddaughter, Rachel Pride of Hubbardston, MA also transitioned after a courageous six-year battle with ovarian cancer. Though the family is heartbroken by these immense losses, they find comfort in knowing Susan and Rachel have taken this journey together.

Private funeral arrangements are under the direction of the Holt Funeral Home, 510 South Main Street, Woonsocket, RI 02895. In lieu of flowers, memorial contributions may be made to Community Care Alliance, PO Box 1700, Woonsocket, RI 02895.
